DOSAGE AND INDICATION
DOSAGE FORMS AND STRENGTHS
ULTRAM (tramadol hydrochloride) Tablets – 50 mg are white, capsule-shaped, coated tablet imprinted “ULTRAM” on one side and “06 59” on the scored side.
Start ULTRAM at 25 mg/day and titrated in 25 mg increments as separate doses every 3 days to reach 100 mg/day (25 mg four times a day). Thereafter the total daily dose may be increased by 50 mg as tolerated every 3 days to reach 200 mg/day (50 mg four times a day). After titration, ULTRAM 50 to 100 mg can be administered as needed for pain relief every 4 to 6 hours not to exceed 400 mg/day.
Dosage Modification in Patients with Renal Impairment
In all patients with creatinine clearance less than 30 mL/min, it is recommended that the dosing interval of ULTRAM be increased to 12 hours, with a maximum daily dose of 200 mg.
Dosage Modification in Patients with Hepatic Impairment
The recommended dose for adult patients with cirrhosis is 50 mg every 12 hours.
HOW TO USE
Take this medication by mouth as directed by your doctor, usually every 4 to 6 hours as needed for pain relief. If a patient misses a dose, they should take their next dose as soon as they remember.
Adult:
Use lowest effective dose for shortest duration. Individualize. Less than or equal to 17 yrs: initially 25 mg every morning; increase by 25 mg increments every 3 days up to 100 mg/day (25 mg four times daily). Total daily dose may then be increased, as tolerated, by 50 mg every 3 days up to 200 mg/day (50 mg four times daily). Usual dose: 50-100 mg every 4-6 hours as needed; max 400 mg/day.
Elderly (greater than 75 years): max 300 mg/day. Renal impairment: max 100 mg every 12 hours. Cirrhosis: max 50 mg every 12 hours
Children:
Less than 17 yrs: not recommended.

CONTRAINDICATIONS
- Ultram should not be administered to patients who have previously demonstrated hypersensitivity to tramadol, any other component of this product or opioids.

- Ultram is contraindicated for all children younger than 12 years of age.
- Ultram is also contraindicated in patients with Significant respiratory depression.
-ULTRAM should not be administered to patients who have previously
demonstrated hypersensitivity to tramadol, opioids or to any component of this
product.
-ULTRAM is contraindicated in any situation where opioids are contraindicated,
including acute intoxication with any of the following: alcohol, hypnotics, centrally
acting analgesics, opioids or psychotropic drugs. ULTRAM may worsen central
nervous system and respiratory depression in these patients.
The concomitant use of ULTRAM and MAO inhibitors (or within 14 days following
discontinuation of such therapy) is contraindicated.
Ultram is also contraindicated in:
-Children less than 12 yrs.
-Post-op management in children less than 18 yrs following tonsillectomy and/or adenoidectomy.
-Significant respiratory depression: Acute or severe bronchial asthma in an unmonitored setting or in the absence of resuscitative equipment.
-Known or suspected GI (gastro intestinal) obstruction, including paralytic ileus.
Ultram Warnings and Precautions
This drug may make you dizzy or drowsy. Do not drive, use machinery, or do any activity that requires alertness until you are sure you can perform such activities safely. Avoid alcoholic beverages.
-Risk of life-threatening respiratory depression and death related to ultra-rapid metabolizers of Ultram (especially in children for post-tonsillectomy and/or adenoidectomy pain).
-Avoid in adolescents 12-18 years with conditions associated with hypo-ventilation.
